went to a specialist once, he had a nice spacious office on 5th Ave, the doctor kept answering his cell phone during my appointment, told me a bunch a crap that didn't sound quire right, and at the end ordered a load of expensive tests that would have cost the insurance company a pretty penny. I decided to go for a second opinion instead. the second doctor was affiliated with the same hospital (department chief), was more experienced, and had actually been voted top doctor in his field... his office was old and decrepit, and I had half a mind to leave while I was waiting. but I was glad I didn't. the doctor spent what felt like an hour talking (and more importantly listening) to me, then he examined me, ordered relatively inexpensive blood tests, and gave me a bunch of probiotic samples. when I mentioned that my medical insurance was due to expire, he reassured me that he would not leave me. quite frequently, all that more expensive really means, is that the doctor has a more expensive office to pay for. I had a similar experience with a different specialist, who dropped me when my insurance company refused to pay his outrageous fees.
